As long as you remember the full name and email associated with the appointment, you can change the appointment as many times as needed. If you wish to cancel the appointment for any reason, just tell me so in the questions and comments box below.
When changing an appointment, fill out the form below as accurate as possible using the full name and email the appointment was made under. Please refrain from using shorthand and abbreviations, offensive or inappropriate language.
